C.O.A.S.T. Through Pregnancy

C.O.A.S.T. (Community Outreach and Support Team) Through Pregnancy is a FREE program by the Capital Area Healthy Start Coalition. It is designed to support pregnant and postpartum women who are at risk for experiencing poor birthing outcomes or postpartum complications due to high blood pressure, diabetes, obesity, or a mental health concern.

Through this program, we want to ensure mothers stay healthy and safe during this crucial period by working closely with community agencies to provide blood pressure monitors, glucometers, scales for health condition monitoring, and a range of services.

C.O.A.S.T Through Pregnancy supports at-risk pregnant and postpartum women through monthly telehealth visits with pregnancy navigators. They provide guidance and resources for a safe and healthy care process, from pregnancy to postpartum.

The C.O.A.S.T. Through Pregnancy program offers:

  • Blood pressure monitors, glucometers, and scales for health condition monitoring.
  • Educational information on health conditions.
  • Screenings for social determinants of health and connecting clients with eligible resources.
  • Assisting clients with medical needs and community resources.
  • Connecting clients with doulas and mental health services as needed.

Don’t navigate your pregnancy alone, sign up for COAST!
