C.O.A.S.T. Through Pregnancy

C.O.A.S.T. (Community Outreach and Support Team) Through Pregnancy is a FREE program by the Capital Area Healthy Start Coalition. It is designed to support pregnant and postpartum women who are at risk for experiencing poor birthing outcomes or postpartum complications due to high blood pressure, diabetes, obesity, or a mental health concern.

Through this program, we want to ensure mothers stay healthy and safe during this crucial period by working closely with community agencies to provide blood pressure monitors, glucometers, scales for health condition monitoring, and a range of services.

C.O.A.S.T Through Pregnancy supports at-risk pregnant and postpartum women through monthly telehealth visits with perinatal navigators. They provide guidance and resources for a safe and healthy care process, from pregnancy to postpartum.

The C.O.A.S.T. Through Pregnancy program offers:

  • Blood pressure monitors, glucometers, and scales for health condition monitoring.
  • Educational information on health conditions.
  • Screenings for social determinants of health and connecting clients with eligible resources.
  • Assisting clients with medical needs and community resources.
  • Connecting clients with doulas and mental health services as needed.

The Community Outreach and Support Team (C.O.A.S.T.) program now includes dedicated Perinatal Navigators to provide personalized support and resources during your pregnancy and the first year postpartum.

What is a Perinatal Navigator?

A Perinatal Navigator is a dedicated professional who offers personalized support and resources throughout your pregnancy and up to one year postpartum. They serve as a consistent point of contact, helping you navigate healthcare systems, access necessary services, and ensure both you and your baby receive comprehensive care.

Services Offered by Perinatal Navigators:

  • Ongoing Condition Monitoring: Assistance with tracking health conditions such as high blood pressure, diabetes, and weight management.
  • Medical Supplies & Education: Provision of essential medical equipment like blood pressure monitors and glucometers, along with guidance on their use.
  • Insurance Navigation: Help in understanding and managing insurance processes to ensure coverage for necessary services.
  • Mental Health Resources: Access to counseling and support services for mental well-being.
  • Health Education: Information on maintaining a healthy lifestyle during and after pregnancy.
  • Lactation & Doula Services: Connections to breastfeeding support and doula care.
  • Depression Screenings: Regular assessments to identify and address signs of postpartum depression.
  • Newborn Care Education: Guidance on infant care practices.
  • Breastfeeding Education: Support and information for successful breastfeeding.
  • Trimester-Based Incentives: Motivational rewards to encourage engagement in prenatal care.

Support Timeline:

  • Monthly Check-Ins: Your navigator will maintain brief contact with you once a month throughout your pregnancy and for up to one year postpartum.

Meeting Options:

  • In-Person Visits: Meet at your OB office for personalized support.
  • Telehealth Services: Access support from the comfort of your home.
